In addition, for 30 days post visit, you have access to our Care Team 24 hrs a day for medical questions or concerns regarding your recent visit, your chronic medical conditions or to discuss any new medical problems you encounter. Additionally, Following your visit, Care2U offers coordination of care back to your PCP (Or Care2U will help find an in network provider if you do not have a PCP), share and discuss results with your Doctor or Specialist at your request, and when applicable, enroll you in chronic care management when services are available.
