As simple as it sounds, exceptional care at your door.

A Care2U Medical Professional arrives at your home within 2-4 hours of receiving your call and verifying insurance.
Our team checks your vital signs, reviews your medical history, conducts a physical exam, and assesses your needs.
A Care2U ER or Hospitalist Physician collaborates jointly through Telehealth to create a treatment plan and ensure a smooth transition to your primary care and/or community care team.
For patients who require in-home hospitalization (and meet the eligibility criteria), Care2U provides 1-2 visits per day, remote patient monitoring, and access to our 24/7 care command center on-call physicians.
Care2U provides care coordination throughout your patient journey..  At the point of discharge from Care2U, our team will seamlessly coordinate care back to your primary care physician and/or community care team.

We understand that when you or a loved one needs medical attention, it's important to get the right care, right away.

How does Care2U coordinate with my primary care doctor or other specialists?

Care2U believes in collaborative care. We work closely with your primary care doctor and any specialists involved in your care. We provide them with regular updates on your condition and treatment plan, ensuring seamless coordination and continuity of care. 

What happens if my condition worsens while I'm receiving care at home?

Your safety is our top priority. Our team closely monitors your condition (by incorporating Remote Patient Monitoring technology), and we have protocols in place to respond quickly if your condition changes. We're prepared to escalate care if needed, and we'll coordinate seamlessly with emergency services to ensure you receive the appropriate level of care without delay.

How do you ensure the quality of care I receive at home is truly comparable to what I'd get in a hospital?
