No. We work with contracted and non-contracted practices. All referrals are welcome and we are an in-network provider. We accept most insurances with the exception of FFS Medicaid (we do accept Managed Medicaid and MLTC).
Our referral process is simple. To make a referral we require basic demographics and a chief medical complaint. We work with your patient directly to obtain more detailed information required for at-home visit including insurance.
Patient safety is our top priority. Our team provides 24/7 clinical oversight for HIPLAC patients and has clear protocols for escalation. If a patient's condition deteriorates beyond what can be safely managed at home, we will coordinate a prompt and safe transfer to the nearest preferred Emergency Department, communicating with you throughout the process.
Yes—Care2U welcomes referrals from specialists. When you refer a patient for the first time, our team will contact your office to understand your communication preferences. Based on those preferences, we can have our attending physician call you after your patient’s visit, speak with your practice manager, or simply fax over the encounter summary. We prioritize collaboration and transparency, ensuring you remain informed and connected to your patient's care.
Care2U is not appropriate for patients experiencing symptoms of a heart attack or stroke, pregnant women, individuals who require blood transfusions, or patients who have fallen, struck their head, and are currently taking blood thinners. These situations require immediate hospital-level resources and should be directed to the nearest emergency department.
After receiving a referral and confirming eligibility/insurance, our team is typically on-scene in the patient’s home within 2-4 hours.
We manage patients with acute needs that, without intervention, would likely result in an ER visit or hospitalization. This includes conditions like COPD or CHF exacerbations, pneumonia, cellulitis, complicated UTIs, dehydration, and other situations requiring urgent medical assessment and treatment such as IV therapies, STAT labs, or mobile imaging at home. We also offer a full in-home hospitalization program (known as HIPLAC) for patients meeting clinical criteria..
