How quickly can Care2U see a patient?
After we receive the referral and confirm coverage, a Care2U clinician is typically in the patient’s home within a few hours. New referrals are accepted 9 AM to 9 PM, every day.
Who comes to the patient’s home?
A Care2U mobile provider, a nurse practitioner, physician assistant or paramedic, arrives at the home, takes vital signs, examines the patient and takes a history. They then start a telehealth session with one of our ER or hospitalist physicians, and the two treat the patient together.
Does my organization need a contract with Care2U to refer?
No. We accept referrals from contracted and non-contracted practices and organizations, and we are in-network with most major insurance plans. We do not accept fee-for-service Medicaid; we do accept managed Medicaid and MLTC plans.
What if the patient gets worse?
Our team manages any change in condition in real time. If a patient is no longer safe at home, we escalate care to the nearest preferred emergency department and keep the referring team informed.
Do you offer inpatient-level care at home?
Patients who meet MCG criteria can enroll in HIPLAC (High-Intensity, Physician-Led At-Home Care): inpatient-level care at home with one to two visits a day, at least one in person, remote patient monitoring, and 24/7 access to our care command center and on-call physicians.
Which patients should go to the ER instead?
Care2U is not the right fit for symptoms of a heart attack or stroke, pregnancy, patients who need a blood transfusion, or a fall with a head strike in a patient on blood thinners. Those patients need an emergency department.
How do you work with the patient’s primary care doctor?
Care2U provides episodic acute care, not primary or longitudinal care. We stabilize the patient at home and send them back to their own doctor. Based on the preferences we collect at your first referral, our physician calls you the same day or sends encounter notes, and you receive the full notes by the next day by fax or email.
