Refer a patient

Refer a patient in a few minutes.

Send the referral online or call intake. A Care2U clinician is typically at the bedside within a few hours, with a physician treating alongside them by video.

Partner referral forms

Use your organization’s form.

Each form routes straight to our intake team with your organization attached.

What to send

We need very little to get started.

  • Patient name, date of birth and phone number
  • The chief complaint. “Unknown” is fine.
  • A contact person, if different from the patient
  • Recent notes, labs or an insurance card, if you have them
After you refer

What happens next.

  1. Intake calls the patient

We confirm coverage and set an arrival window.

  2. Care at the bedside

A clinician arrives within a few hours, and a physician treats by video.

  3. Same-day update

Our physician calls you, or we fax the summary, based on your preference.

  4. Back to you

Full notes the next day, and the patient returns to their own doctor.

FAQ

Referral questions, answered.

Does my organization need a contract with Care2U to refer?

No. We accept referrals from contracted and non-contracted practices and organizations, and we are in-network with most major insurance plans. We do not accept fee-for-service Medicaid; we do accept managed Medicaid and MLTC plans.

What do I need to send with a referral?

Basic demographics and the chief complaint. Our intake team contacts the patient directly for insurance and anything else we need for the visit.

Which patients should go to the ER instead?

Care2U is not the right fit for symptoms of a heart attack or stroke, pregnancy, patients who need a blood transfusion, or a fall with a head strike in a patient on blood thinners. Those patients need an emergency department.

Can specialists refer patients?

Yes. At your first referral our team asks how you want to hear back: a call from our attending physician, a call with your practice manager, or the encounter summary by fax.

Where does Care2U operate?

In New York: Manhattan, Brooklyn, Queens, The Bronx, Staten Island, Nassau, Suffolk, Westchester and Putnam.
