Hospital-level care, without the hospital.

Care2U works directly with PCPs and Care Managers to provide on-demand care, stabilization of urgent needs, initiation of treatment, and seamless coordination back to the patient’s primary care team.

More control over care, more transparency into treatment.

Care2U monitors your patients for a full 30 days and evaluates high-risk escalations against MCQ/IQ criteria, all while providing daily Care Summaries and comprehensive Discharge Plans.

Trust is your patient’s primary comfort.

Care2U collaborates with Hospital Discharge Planners to initiate transitional care prior to discharge, visits your patients at home 24-48 hours following, and administers, monitors, and evaluates healing in partnership with their fuller care team.

Same care, better quality, lower cost.

Care2U works with treatment administrators to determine if your patient requires more intensive SNF-level care; bringing in skilled clinicians accordingly to help with decreasing length of service and safely transitioning care back into their home.

“What’s the catch?” is just one of the first questions asked after learning about the Care2U Platform but, in case you have others, feel free to get in touch or skim through these FAQs:

How do I enroll my patient into the Care2U program?

As a Physician or Care Manager, you’ll be assigned a dedicated number that allows you to easily call a Care Coordinator and set up a same-day, in-home evaluation followed by a treatment planning discussion with a member of the Care2U Clinical Team.

Can Care2U really provide inpatient-level care in the home?

If your patient meets Milliman or Interqual Criteria for hospital admission, yes, Care2U can provide inpatient-level care right in their home.

What are Care2U’s clinical capabilities?

Rapid, same-day, in-home evaluation,  stat labs, stat X-Ray, stat ultrasound, IV infusions, home oxygen, DME, hospital beds, RPM, homecare attendants, OT/PT, or social workers as needed.

How involved do I need to be as the Physician or Care Manager?

After making the referral, you and the Care2U Clinical Team will discuss your patient’s care plan; upon your patient’s discharge from the program, the Clinical Team will provide you with thorough admission summaries and arrange follow-ups at your – and your patient’s – convenience.

Who is liable for my patient’s outcome after making the referral?

Care2U assumes all liability for patient care upon intake through discharge of the program.

What if my patient decompensates?

Any status change is managed in real-time, and if your patient’s condition is no longer appropriate for in-home care, their care plan will be escalated to a hospital transfer: with stable patients being transferred to a hospital of their choice, and unstable patients being transferred to the nearest 911-receiving hospital.

Care2U bills your patients’ insurance providers directly, and many often have reduced copayments/deductibles compared to traditional inpatient admissions.
