  • A Care2U ER or Hospitalist Physician collaborates to create a treatment plan and ensure a smooth transition to your PCP’s care or an appropriate follow-up program.

Who comes to the home?

The Care2U program is supported by a multidisciplinary medical team of mobile care specialists, nurse practitioners, and physician assistants providing care in the home while ER & Hospitalist Physicians collaborate remotely via telehealth.

What can Care2U treat in-home?

Care2U delivers most clinical capabilities of a traditional skilled care setting, treating acute medical conditions such as UTIs, dehydration, pneumonia, cellulitis, COPD, and CHF exacerbation in the comfort of the home. Clinical capabilities include IV fluids/ medications, in-home X-rays and ultrasounds, same-day labs, oxygen, wound care, and so much more.


How long does it take Care2U to come to my home?

Care2U is there within 2-4 hours of receiving your call and verifying insurance, but you’ll be assigned an arrival window immediately, and will be kept updated when our team is enroute.

Does Care2U accept insurance?

Care2U sees all patients regardless of insurance type. Standard copays and deductibles apply.

Do you coordinate with my Primary Care Physician?

Yes, Care2U coordinates directly with your primary care physician and community care team, ensuring that any updates relevant to your care are provided to the respective parties, and always with your permission.

Care2U bills your insurance provider directly and many often have reduced copayments/deductibles compared to traditional inpatient admissions, but coverage may vary by payor and plan.
